YOU ARE HEREBY NOTIFIED THAT  you have violated the terms of the lease for the premises situated in the City of [Tenant's City], County of [Tenant's County], and State of Illinois, being described as follows: [Tenant's Street Address].  You have violated the lease by [List of lease violations].

(This paragraph only applies to a property that is governed by the Chicago Residential Landlord Tenant Ordinance.) And you are further notified that your tenancy and lease is terminated as of [DATE]. You are hereby required to surrender possession of said premises to the undersigned on that date, unless the breaches, as described in this Notice, are remedied by you within that period of time, and provided that the breaches can be remedied.

No further demand shall be necessary before bringing legal proceedings to recover the premises.
